Salary: £36,153.00 - £39,961.00 per annum (Grade 10)

Due to an internal restructure within the Edge of Care service we are recruiting to the position of full time Team coordinator based in Stafford, but working across the county.
This will be a 12 month fixed term contract or secondment opportunity for people presently working for Families First.
The Edge of Care service incorporates the Intensive Prevention Service which went live in January 2013.
The service has been successful in keeping young people at home and improving their outcomes and has expanded over time to support young people returning from local authority care
The service is based around a model of intensive one-to-one key work over a sustained period targeted to meet the specific needs of the client.
The key worker will then be responsible for working with young people and their parents to develop a tailored package of support, drawing in and coordinating other resources within the team as appropriate. The key worker will also support young people and parents to identify and engage with any relevant external support services as required e.g. substance use services, housing providers etc.
You will have responsibility for managing referrals, allocations, quality and performance within the service.
You will have case management responsibility for a small number of cases.
You will need a Social Work Qualification and have at least two years post qualification experience in Children’s services.
