The postholder will be required to work closely with older adults with care and support needs, carers, and other health and social care professionals, providing an assessment and care management service. The social work post will be based within Older Adults Services which covers intake function, case management , reviews and will involve working in partnership with colleagues and staff from other agencies.

The postholder will be required to comply with relevant legislation, including the Care Act, the Mental Capacity Act, Mental Health Act and Data Protection Act . Postholders will be expected to promote strength based social work, and support people to contribute to their community.

You’ll need:
* A qualification in social work
* Current registration with HCPC
* Knowledge of assessment models, case management and reviewing processes.
* Understanding of a strength based approach to assessments and support.
* A high level of organisational skills
* Effective communication skills, including written and IT skills
* Excellent interpersonal, negotiation, problem solving and analytical skills

This role is based within a multi-disciplinary team, within Adult Social Care, part of the People Directorate. We aim to support people to live independent and fulfilling lives, free from harm or abuse. The successful candidates will be based at the Friargate headquarters which is a prestigious newly constructed office building, close to Coventry train station.Friargates houses a variety of services, and is occupied by around 1,800 staff working in an agile manner.
