Job title: Wellbeing Officer

Team: Wellbeing

Hours: 37 per week + working on a duty rota in order to cover the operating times of the service which are 8:00am – 8:00pm over 7 days per week.

We are looking for individuals who are ready to take up an exciting and challenging role within our Wellbeing Service. The person we are looking for will be an excellent communicator, have a good understanding and/or experience of Social Care or Support Services, good IT knowledge, be organised, motivated and a have a passion for working with elderly and vulnerable people.

As a Wellbeing Co ordinator you will specialise in support and prevention services. Working alongside partners you will provide a range of services that develop and sustain an individuals’ capacity to live independently in their own homes. This includes Trusted Assessments, Assistive Technology & Telecare, Community Alarms, Emergency responses & Welfare Checks and Home Support services.

Solihull Community Housing has a responsibility for, and is committed to, safeguarding and promoting the welfare of children, young people and adults at risk, and requires all staff and volunteers to share this commitment.

This post will work alongside partners to ensure that older and vulnerable members of the community maintain their independence.

To work within a dedicated team providing a range of services that focus on people’s wellbeing and support them to stay independent for as long as possible.

Provide support functions that will help guide people to the most appropriate equipment/services/advice available in a holistic, person centred way

Provide support services that develop and sustain service users’ capacity to live independently in their own homes, thereby reducing the need to move to higher-dependency accommodation.

Using the Trusted Assessor Framework, the post holder will advise on and deliver measures that support independent living through a range of interventions including

• Assistive Technology and Telecare

• Aids and adaptations

• Information and advice to ensure people have the means and information to support them with their lives

• Practical support to live safely at home ( Home Hazard Assessment, Handyperson service)

• One to one support – realistic options to engage with other people in the community, avoiding social isolation

• Signposting to other appropriate services and opportunities

The aim of the service is to offer a flexible menu of options that are fully responsive and customer focussed reaching a wide client group which will include Hospital to Homes services. You will be required to work a duty rota in order to cover the operating times of the service which are 8:00am – 8:00pm over 7 days per week.

• To undertake Wellbeing Assessments in line with the Trusted Assessor framework

• To develop a person centred Life Plan to identify a person’s needs and the help and support they need to continue to live safely and independently in their own home

• To develop and maintain effective working relationships with other professionals involved in that persons support

• To liaise with a wide range of statutory, voluntary and third sector organisations to access services activities and opportunities

• To monitor progress and long term inclusion of clients and their wellbeing

• To work with an identified caseload to assess and agree programme interventions to enable service users to achieve goals identified through the Outcome star process.

• Ensure people have greater choice and control over meeting their needs

• Champion Health and Wellbeing. Working closely and collaboratively with colleagues, partner organisations and community groups, develop a portfolio of preventative community based initiatives and interventions

Experience of working with vulnerable people with diverse needs

Experience of group development and delivery

Experience of individual needs assessment

The ability and commitment to be flexible with regard to working hours, accepting that on occasions the role may require extended hours or out of hours working

Ability to drive and with access to own vehicle
