This is a exciting opportunity to work with service users/families in crisis resolution in a multi agency approach from a mental health perspective.

Hours:  37 hours - Permanent

Location: Ladies Walk Centre, Ladies Walk, Sedgley. DY3 3UA

Based within the Dudley Disability Service offering evidence-based treatment within a recovery model in order to improve service user independence and access to community care. The service will increase stability in the lives of service users and their carers by providing innovative quality services and prompting autonomy.

The post holder will care co-ordinate a caseload of service users with complex mental health needs and must hold a current AMHP Approval.  They make use of sophisticated, critical reasoning to facilitate reflective and evidence-informed practice.

We also support people with learning disabilities, autism/higher functioning autism and people who have significant multiple and complex presenting problems/needs or challenging behaviours that place the person, their staff/carers or the community at risk, that fall outside of the remit and are not currently met by other services/partners.

Post holder will work closely with Black Country Partnership Foundation Trust in the assessment, co-ordination and management of care for clients identified via the Transforming Care Programme.

There is an expectation to supervise students and Assistant Care Co-ordinators.
