Staffordshire is looking to recruit a Special Educational Needs and Disability Key Worker to work within SEND assessment and planning team .The team is part of Families First that brings together the full range of front - line services that deliver to children, young people and their families. The Focus of the post is the co- ordination and implementation of Education, Health and Care Plans from 0-25   .The post is based in the district team in Burton on Trent.  

We are seeking  a passionate, motivated individual who has  a real drive to work with children and young people to co-ordinate new assessments in a truly person centred way that fully involves the child, young person and their family in this process.

Travel to other areas of Staffordshire will also be required.
