Learning Disability Social Care Practitioners. 
An exciting opportunity has arisen for a dynamic, committed, passionate and caring Social Care Practitioner to join our Learning Disability Service within Warwickshire. The team is currently based in Warwick and Bedworth and consists of Social Care Practitioners, Social Workers, Occupational Therapists and Occupational Therapist Assistants.

The Role

You will be required to work with a variety of customers, including people with Learning Disabilities and Autism Spectrum Disorder (ASD) with physical disabilities, mental health; carers and their families and other professionals.
You will be required to undertake outcome focussed assessments, support planning and reviews. Focussing on strengths, you will enable customers to identify outcomes, meet their full potential and maximise their independence. As a strong team player, you will be required to contribute to the development and promotion of the Learning Disability Team, using imaginative, creative and innovative ways of meeting the support needs of adults with Learning Disabilities. The roles will give the applicants the freedom to think creatively and design support plans tailored to individual’s support needs and outcomes. 

Key Requirements

You will need experience of working with people with Learning Disabilities and / or Autism, and be committed to improving the experience and outcomes of adults with Learning Disabilities. You will have the opportunity to provide a first class service to adults with Learning Disabilities, carers and their families by helping develop and utilising local community services.
