Service Delivery Manager: Adult Social Care Prevention and Enablement

£59,063 - £62,056 per annum

Grade: SMG 3

37 hours per week

In Telford and Wrekin we aim to provide excellence in service delivery for those with care and support needs that we serve. We are proud of our collaborative working and integrated approach with partners to avoid hospital admission unless absolutely necessary and supporting discharge  and re-ablement for those that do have a hospital stay.  Our Performance is good in this area but we are committed to continual improvement. We pride ourselves on promoting independence at every level by using a strengths and asset based approach, alongside therapeutic interventions and digital innovations that promote well-being.

Due to the retirement of our current Service Delivery Manager we are looking for a stellar individual to lead our Integrated Support Service in Telford and Wrekin Adult Social Care. This post leads the integrated hospital discharge service, occupational therapy service and assistive technology and sensory impairment service within Adult Social Care.

 

This is a key senior leadership post within Adult Social Care, with innovation and partnership working at its heart. We are looking for an experienced visionary and innovative leader, with a professional social care or health qualification who has the drive, passion and tenacity to lead and further develop the prevention and enablement aspect of our service.

As part of the Adult Social Care senior Leadership Team you will also contribute to the development and delivery of transformational projects to enhance our service offer across all of Adult Social Care.
