£23,866 - £25,463 per annum
Working pattern: Shift work. Early and late shifts which includes working weekends and all bank holidays. Sleep ins are required


We are seeking to appoint an Outreach Support Worker to support young people who are Looked After and are living in supported living accommodation. The Team will provide support to young people who are making transitions from family or residential placements or who are already living independently. The Outreach Team also provide support to children and families, to prevent placement or family breakdown and to enable successful reunifications.


The role is 37 hours per week and requires working in partnership with relevant professionals and agencies, to ensure good outcomes and life chances for young people in Worcestershire. You will be required to assess, plan and review the needs of young people in order to ensure that their needs are accurately assessed, and that support is in place to meet their current and future needs.


Within the role it is essential that you have experience of working with young people and the ability to develop positive relationships. You must have the ability to understand and work with young people, who have a range of social and emotional difficulties and who can present complex and challenging behaviours.


The role involves providing support in emergency and crisis situations to young people across Worcestershire, which includes working evenings, weekends, shift work and all Bank Holidays. We are looking to appoint staff who are committed and skilled in working with young people and who can empower young people and families to develop positive change within their lives.
