2 Full Time posts - Fixed Term of 24 months.
Working Pattern – 37 hours over 6 days (Mon-Sat)
Salary Band – Band E (£28,941 - £36,446) Secondment Opportunities Available

Are you a health / social care professional with a passion for caring for adults and desire to see their smooth transition from hospital to care home? Do you have proven leadership skills in either a health or social care setting?
These exciting new posts are a pilot to support and facilitate effectively and timely discharges from the acute and community hospitals into Care Homes by working as an independent trusted assessor. These roles will be the liaison between the acute and community hospital setting and care home to ensure a smooth transition.
The post holders must hold a care/ health professional qualification and be able to:
• Undertake assessment / reassessment on behalf of residential and nursing homes
• Demonstrate experience of undertaking complex assessments and working within a multi-disciplinary / organisational settings
• Maintain accurate and high quality records of Trusted Assessments
• Build ongoing relationships with the local care economy to promote the role of “Trusted Assessor”
• Develop a Trusted Assessor Pathway and generic assessment format that is acceptable to be used in social care, acute / community hospitals and care homes
• Maintain open communication with social care providers and commissioning to identify potential sources of delay and make recommendations for improvements to support effective outcomes for adults
• Develop and gather information to demonstrate effectiveness of Trusted Assessor role. This will include the production of monitoring reports.
