Job Details

Warwickshire County Council is embarking on an exciting programme of work with health, social care and care provider partners. The aim is to improve patient flow through hospital and assist with reducing Delayed Transfers of Care.

The Role

37.0 Hours per Week
Fixed term contract - 12 months

Feedback has shown that at times many residential and nursing care home providers find it difficult to carry out timely assessments within hospital settings, which enable individuals to safely discharge from hospital into a care setting.

In order to support this, we are recruiting for two Trusted Assessors across Warwickshire. Following a period of inpatient care and following completion of a social care practitioner’s statutory assessment, the Trusted Assessors will work in close collaboration with multidisciplinary discharge teams and care home providers to complete an assessment of need on hospital patients. 

We are looking for two Trusted Assessors to support the Hospital Social Care Team and Warwickshire’s care market to ensure that those over 65 return home to their residential or nursing home without delay. 

The successful post holder will be required to: 


• Carry out assessments on behalf of residential and nursing homes; 
• Liaise with homes, hospital social workers and hospital discharge teams; 
• Play a key role in reducing delayed transfers of care from Warwickshire’s acute hospitals (Warwick, George Eliot and UHCW) 
The Trusted Assessors will be based (or will visit patients) at one of these three acute hospitals in Coventry and Warwickshire: 
• Warwick Hospital (South Warwickshire Foundation Trust) 
• George Eliot Hospital 
• University Hospital Coventry and Warwickshire

The successful candidates will be required to contribute to the development and evaluation of the effectiveness of the Trusted Assessor scheme, and work closely with all key partners, including health, social care and the residential and nursing care market to embed the scheme and ensure all are fully involved and invested in the development of the service. The successful candidates will be familiar with developing new services, and will be able to enhance the scheme over time, including demonstrating the benefits and encouraging other providers of residential and nursing care to utilise it.

 

Key Requirements

• Experience of assessing service users’ needs within a health and/or social care environment. 
• Experience of working with individuals with complex health and social care needs, including long term conditions, complex comorbidities and/or dementia. 
• Proactive and able to use own initiative. 
• Excellent written and verbal communication skills, including experience of working with a range of stakeholders including service customers, health and social care professionals. 
• Excellent eye for detail and ability to extract, analyse and interpret relevant information from multiple sources, including qualitative and quantitative data. 
• Effective negotiation skills, experience of resolving conflicts and undertaking challenging conversations while maintaining professionality. 
• Problem solving skills including the ability to work innovatively develop plans and table solutions. 
• The ability to work in high pressure environments.
