Dudley Council has been transforming the way it delivers adult social care and is investing in a multi-million pound package of adult social care and health initiatives for the most vulnerable.

The council has been awarded £13.9million to spend over the next three years as part of the government’s £2billion Improved Better Care Fund with the majority being used to introduce a series of initiatives to help meet the increasing demands on adult social care and reduce pressures on the National Health Service.

We are now looking to recruit a number of posts on a fixed term basis to help support this transformation of supporting our ageing and disabled population.

We are looking to recruit a number of Assistant Care Co-ordinators who have proven assessment experience. The roles will be to support:
• Hospital discharges
• Prevent admission to hospital
• Hospital front of house / Emergency response team
• Promote and enhance personal performance through rehabilitation
• Review care packages and maximise the use of Single handed care.
• Review the provision active hoists and other moving and handling assessments.

A knowledge in any of the following areas will be beneficial; equipment and adaptations, rehabilitation, moving and handling techniques.

The duration of these posts are minimum 12 months fixed term from start date with the opportunity of extending the contract subject to funding

We are accepting internal applicants on a secondment basis however, you will need the consent of your line manager to apply .
 


The post holders will need to creative in their thinking and be able to problem solve.

The post holders will need to work with the people involved in the care to help with the transition of change. Repetitive visit at times convenient to the care provision is essential.

The posts are to support urgent care, hospital discharges, prevent admission to hospital and to maintain people in the community.

They undertake assessment either at the person’s home or at the hospital with a follow up visits to the person home.

The post holders will set up safe systems of care by obtaining appropriate equipment/ services, link with agencies / carers and complete the appropriate handover of the equipment and/or the method of care.

The single handed care philosophy and a rehabilitation programs will feature predominantly within these posts; to maximise the persons engagement in care and to utilise the rehabilitation agencies to deliver effective programs of support.

We are committed to safeguarding and promoting the welfare of vulnerable adults; therefore the post is subject to medical clearance and an enhanced DBS check.
