The Asylum and Leaving Care team are looking for an Advanced Practitioner to join us. We are a stable and welcoming team based over two sites covering the whole of Warwickshire. We are looking for an Advanced Practitioner social worker who strives towards consistently good practice, with a genuine commitment to getting the best outcomes for our care leavers, through the provision of support as set out in our Local Offer to care leavers and by keeping our young people at the centre of all that we do. We welcome creative, solution focused colleagues with a real drive to supporting our ongoing development as a service. 

The Role

Joining our established team you'll be providing a social work service to a highly professional standard. Managing a caseload and travelling throughout the county, you’ll be working with care leavers who qualify for a service from the Leaving Care team. You will be working closely with care leavers and producing their Pathway Plan alongside the young person. In addition, you’ll be facilitating Pathway Plan reviews for our former relevant care leavers and also supervise a small number of Personal Advisors and facilitate their group supervision sessions. The Advanced Practitioner role gives you an excellent opportunity to build close, effective relationships with care leavers, promote Warwickshire's Local Offer to care leavers and contribute towards the continued development of the Leaving Care service. 

Key Requirements

To be successful in this role it’s important you have a professional Social Work qualification, are registered with the Health and Care Professions Council and have a good understanding of Leaving Care legislation. It’s also important you have strong analytical and communication skills both written and verbal and can work on your own initiative as well as having a track record of building strong relationships with care leavers, colleagues and other agencies. With the ability to work to and meet statutory deadlines, at times under pressure, whilst at all times maintaining composure and your professional stance, you will be used to supporting care leavers through to independence and beyond. With strong influencing, persuading and resilience skills you’ll also have child care experience and good IT skills.
