Team Leader – Urgent Promoting Independence Team
Salary: £26,470 - £28,221
Fixed term contract until 31/03/2020

The Urgent PI Service provides support to individuals who live in their own home. The service is registered with the Care Quality Commission and is based at Wildwood, County Hall Campus. This service is provided across the whole of the county of Worcestershire and teams operate within specific areas.

We are looking to recruit a Team Leader who will manage a staff team, assess people for reablement or on-going care services, liaise with GP's and other professionals and maintain good working relationships within the Adult Services Directorate. The aim of the service is to provide support to individuals so they can remain in their homes and to promote their independence following hospital discharge or crisis intervention and, to observe during a period of assessment any ongoing service requirements that would aid recovery.

The service is provided from 07.00 to 22.00. It is centrally managed through a Call Centre based in Worcester, which is open from 06.30 to 22.00. The post holder would take part in a rolling rota which covers 7 day working.

You should have a commitment to developing the lives of people who have a diagnosis of a dementia, providing care and support that meets individual outcomes and promotes independence. You should have the ability to travel independently, work autonomously and be a good team player. You need to have good communication skills, excellent organisational skills and experience of managing and leading teams. You will need an understanding of maintaining the health & safety and wellbeing of service users in their own home.
