Children with Disabilities Short Breaks Review Worker

Salary up to £25,463

Education Health and Care Plan reviews are at the heart of ensuring that the voice of a child or young person with additional needs is heard throughout any professional intervention.

The Children with Disabilities Team has created an exciting new post to ensure that working alongside education and health partners, social care provision is reviewed in line with the EHCP, and ensure it continues to meet need, and enhances the child or young person's life.

The post holder will have responsibility for carrying out Early Help assessments and reviewing how the needs of our children and young people are met by Child in Need of Short Breaks.

If you have a background in social care, education, or health services, then this could be the role for you.

The post is countywide, based within one of two Children with Disabilities Teams, which offer support, training, regular supervision, and professional development.  It will require attending EHCP reviews, meeting with children and families, and working with multi agency partners.
