Wrekin Locality - Adult Practitioner

£21,074 - £23,111 per annum

Scale 5

37 hours per week

Integral to the post will be working to Support Plan with individuals prior to and/or after a Care Act Assessment.

To provide a broad range of interventions, that will, through close work with and support to, provide a provision that is holistic, evidence based and delivered within communities.

Building strength and confidence for individuals and families to have control of their lives.

Providing or sourcing the support that is needed to develop wellbeing and improve connections to Family, friends and community.

The post holder is integral to assisting the individual and their family to make informed decisions about the level of choice, control and risk they wish to take with their lives.
