Job Details

The role is working for Birmingham City Council's Home Care Enablement Team. You will work as a small team of Care Coordinators based at Lancaster Circus. The operational hours of the service are 6.45 am to 22.50 pm 7 days per week , 52 weeks per role.

The advertised role will require you to work a shift pattern over 7 days per week covering the 14.00pm-22.50pm shifts.

The Post Holder as part of a team of Care Coordinators, will be responsible for the day to day planning, allocation and monitoring of Enablement resources to ensure the effective consistent and reliable delivery of safe , caring and responsive services to Citizens. This includes the day to day allocation of work to care staff, planning and coordination of care, planning the forward rota, responding to changes and events to maintain continuity and effectiveness. The role also includes maintaining accurate information systems so that management and performance information is accurate and up to date

To receive new referrals planning and coordinating care, allocating and maximising the use of resources so that all are responded to within agreed timescales.

To plan the on -going day to day allocation of work to care staff and plan the forward rota , to maximise service quality, continuity and the efficient use of resources .

To ensure the continuity of service provision to citizens in the absence of regular carers due to sickness, leave ,by the use of agreed procedures .

To contribute to ensuring that service delivery values and respects the diversity , dignity and needs of citizens and meets regulatory standards,

Experience of:

Working successfully in a care or programming environment.

Working as part of a team.

Working in a pressurised and changing environment.

Communicating with members of the public, colleagues and other professionals.

Communicate effectively , verbally and in writing to members of the public, colleagues and members of external organisations.

Basic numeracy and literacy skills.

Use of a variety of IT applications.

Able to prioritise and organise a diverse workload and maintain a safe , effective and consistent service.

Understanding of social care and what standards are required to deliver a safe , effective , caring and responsive service.

Will work as part of a 7 day rota covering 14.00pm - 22.50pm 52 weeks of the year. It is an essential requirement of the job that post holders work flexibly.
