The key focus of this role is to coordinate Herefordshire Council’s urgent care service, to ensure there are reduced delays in the discharging of patients from hospital and adults within the urgent care pathway.

You will track the flow through the service, highlighting delays and work collaboratively with both internal and external service providers and partners, to ensure discharges are timely and efficient, leading to an improvement in health outcomes for patients.

You will be responsible for driving forward service delivery by having continued commitment, determination and patience, working with every team and linking teams together to find solutions.

You will also be responsible for all monitoring and reporting processes involved within urgent care.
Key skills require include having a sound knowledge and understanding of social care services and operation, experience of working in a social care setting and the ability to extract, manipulate and present data.
