We have a fantastic opportunity for an experienced, family support worker who is passionate and committed to providing targeted support to children and families; with a good understanding and knowledge of child development and adolescence social, emotional and mental health needs.

Targeted Services Support based within the COG work with Children and families that are at risk of family breakdown, have complex needs with other contributing factors i.e. drugs, mental health, parenting or long-term involvement with services. The aim of the service is to prevent an escalation into specialist services and prepare young people to make a successful transition into adult life.

About the Role:

The Targeted Family Support Worker is an exciting and unique role, holding a caseload of complex cases within a highly supportive environment. With post qualifying experience of successfully managing a caseload of complex cases, completing Early Help Assessments, direct with children and parents/carers to prevent escalation to statutory services.

Working within a multi-disciplinary team you will work in partnership with staff and other colleagues from both the voluntary and statutory sector to support children and young people, who would be less likely to achieve or have the opportunity to achieve a successful transition into adult life.
