Full – time

An exciting opportunity has arisen for the above positions. You will work closely within a Multi-Disciplinary Team to provide Statutory Social Work. You will work with individuals within the acute hospital supporting the Discharge to Assess approach and act as community worker for those transferred home or within community bed based rehabilitation and enablement after hospital care or admission avoidance

Using care closer to the community principles, these posts will work with a wide range of individuals who have will have had a period of hospital stay or to prevent hospital admission. You will be a key player in maintaining strong partnerships with health and third sector colleagues to promote a community and asset based approach aimed at maintaining people’s independence in their own homes; minimising the impact de-compensation which affects both physical and mental wellbeing.

You will undertake statutory social work in line with the Care Act, Mental Capacity Act and

Mental Health Act. You will undertake duties expected of social workers in line with the Professional Capabilities Framework.

You need to have a minimum of 2 years' experience and be expected to work 37 hours over a 7 day period
