An opportunity has arisen to work in the Adult Social Care Brokerage Team to support individuals to access appropriate home care, respite, nursing/residential placements and other social care provision.  

The Brokerage Team arranges the delivery of services in a cost effective manner that meets individuals' intended outcomes as set out in their care and support plans. The team provide a point of reference for the co-ordination and provision of services, liaising with care management teams, finance teams, internal and external providers and are responsible for ensuring that Adult Social Care systems are updated to ensure best value for procuring care and support services in line with agreed processes.

37 hours per week
