Two Posts 37 Hours per week – Permanent

We want people to have dignity and self-respect and for those with more complex needs to have the support they need to maintain this.  

A rare and exciting opportunity has arisen to join the county’s high performing and citizen-focused Adult Learning Disability Team (ALDT) which is part of the Health and Social Care at Staffordshire County Council

Following a successful and positive recent restructure of Staffordshire’s Social Care Offer to its citizens, two new posts of Social Care Practitioners have been created. These posts are in place to support Adult Social Care Service to deliver the statutory care and support offer that the council offers to adults with learning disabilities to achieve their personal outcomes and goals in life as part of . The successful candidate will be an integral part of the ‘Assessment and Care Management’ teams, in the  North  and the South of the county .These teams support adults (18+) with a learning disability and autism who require support from adult social care and with eligible social care needs as defined by the Care Cat 2014. The teams are part of the wider Adult Learning Disability Services within the council. 

The ALDT works alongside citizens their family/carers in carrying out outcome focused reviews/assessments and developing support plans to meet the adult’s assessed eligible needs as defined in the Care Act 2014 to promote independence and meet identified outcomes and aspirations.

We are looking for highly motivated, flexible individuals who have a proven track record of a person centred and asset focused outlook and have high professional standards. The ideal individual will also have knowledge and experience of current legislation and its application to practice in a social care setting. Experience of working in complex situations using professional judgement and a range of interventions. You will be able to communicate in a clear, fluent, concise, jargon-free way and in a courteous, calm and professional manner.
