Job Summary:

Provision of Services to support/assist vulnerable adults with a range of mental health needs.

To support residents with a range of issues which could include but is not exclusive of managing their mental/physical health successfully, maximising their independence (includes budgeting, menu planning, cooking etc), exploring education opportunities and maintaining a presence in the community.

To support the Care Services Manager/Team leader and follow the day to day guidance of the Shift Leader by taking on elements of the duties of the post, as delegated from time to time e.g. attend meetings.

Specific Duties:

  1. To take on duties as delegated by the Manager/Team Leader.
  2. To undertake regular assessments of client need and to identify outcomes in conjunction with the Resident and other partners as appropriate.
  3. To devise individual support plans in partnership with each client (working within the recovery and outcome focussed model of delivery) and to ensure that these are reviewed within agreed timescales and in conjunction with other partners as appropriate. Support plans to be implemented from the commencement of support and reviewed/updated regularly or when outcomes are reached and other outcomes identified.
  4. To provide person centred, practical support and reassurance to enable clients to manage/maintain their mental/physical health successfully, to improve their quality of life and to maintain a presence in the community.
  5. To use whichever outcome measurement tool deemed necessary by the Manager, this may include written or electronic methods of recording.
  6. To complete risk assessments and report any safeguarding issues through the appropriate channels.
  7. To maintain clear and concise, factual notes in relation to the service provided to each client, including recording of every contact with the client together with any duties or contacts made as part of the support package.
  8. To liaise with a range of professionals, including but not exclusive of health and other support providers as appropriate.
  9. To provide the support perspective (as opposed to care perspective) when attending multi-disciplinary service reviews, care management reviews, etc.
  10. To liaise with relevant partners where signs of relapse and/or dis-engagement are apparent.
  11. To develop and maintain a good working knowledge of community resources which may be relevant to the needs of clients, and share where appropriate.
  12. To effectively partake in training relevant to the post, as required by the organisation.
  13. To participate fully in team meetings and to engage in monthly/six weekly supervision sessions with your Manager.
  14. To undertake sleep in duties to cover for colleagues during absences, (sickness/annual leave) as and when required and to ensure that appropriate information is conveyed to colleagues regarding support tasks before planned leave.
  15. To work flexibly and have the willingness to cover at services within the organisations other support services.
  16. To effectively correspond to other members of the support team, managers and directors of KPG, as well as other departments, via e mail and respond to those received accordingly.
  17. To attend all mandatory training and any training deemed appropriate to the role.
