Dudley Council has been transforming the way it delivers adult social care and is investing in a multi-million pound package of adult social care and health initiatives for the most vulnerable.

The council has been awarded £13.9million to spend over the next three years as part of the government’s £2billion Improved Better Care Fund with the majority being used to introduce a series of initiatives to help meet the increasing demands on adult social care and reduce pressures on the National Health Service.

We are now looking to recruit a number of posts on a fixed term basis to help support this transformation of supporting our ageing and disabled population.

Hours:  37 per week

Salary:  Grade 8

We are looking to recruit a qualified Social Worker with a Health and Care Professions Council registration who has experience of working with people with a range of complex needs.

Your role will involve supporting and supervising Assistant Care Co-ordinators with advice and guidance with cases to promote their knowledge and understanding and to ensure they are working within required legislation, e.g. Care Act, Mental Capacity Act.

The Social Worker post will cover one or all of the following:

Improved Discharge flow - Seven days a week service – from the ward or assessment unit

Emergency Response team - Seven days a week service- from the front of house (A&E) preventing admission to hospital

As an exemplary practitioner, you will be required to provide support, advice and share professional knowledge to colleagues; promote innovation and model and facilitate reflective practice.

The post will involve working with adults, some of whom may be vulnerable. You will be required to undertake assessments and develop support plans to enable service users to achieve an optimum level of independence, to develop and maintain relationships and be valued members of their community.

You will need to be able to demonstrate professional commitment and a high level of knowledge, critical thinking, communication skills and creativity, in order to support people to achieve positive outcomes and a good quality of life.

You will be involved in working with multi-agencies to facilitate safe discharge following different pathways as necessary.

You will also be asked to undertake duty and respond to front of house to facilitate immediate discharges for those who are not liable to be admitted.

37 hours will be worked on a set rota to include weekends, evenings and bank holidays for which relevant enhancements will be paid.

You may need to work flexibly, travelling across the Dudley borough and will therefore need to hold a valid full driving licence and have the use of a vehicle.

The duration of this post is up to March 2020.
