Job Details
Sandwell Adult Social Care Health and Well-being are recruiting a Social Worker. This is a borough wide hospital/intermediate care service.
The service aims to improve the outcomes of residents in Sandwell by;
- Enabling/facilitating timely discharges from hospital
- Monitoring and facilitating timely discharges from intermediate care provisions
- Offering alternatives to hospital admissions
- Supporting people to return to independent living
You will deliver an enhanced customer experience that promotes the wellbeing of vulnerable adults to enable them to live independently for longer, supporting them in retaining and regaining their skills and confidence.
You will be social work qualified and hold either the degree in social work, Dip SW, CQSW or equivalent and HCPC registered.
Offering regular supervision, generous annual leave entitlement and excellent employee benefits package.
An Enhanced DBS will be required for these posts.

37 hours per week
