Outreach Support Worker

Full time - Salary up to £26,317

Due to the Outreach Team expanding, we are looking to recruit an additional Outreach Support Worker to support young people who are Looked-After. You will be supporting young people who are aged 16 years to 18 years old. The Team provides support to young people who are making transitions from family or residential placements or who are already living independently.

The role requires working in partnership with relevant professionals and agencies, to ensure good outcomes and life chances for young people in Worcestershire. You will be required to assess, plan and review the needs of young people in order to ensure that their needs are accurately assessed and that support is in place to meet their current and future needs.

Within the role it is essential that you have experience of working with young people and the ability to develop positive relationships. You must have the ability to understand and work with young people, who have a range of social and emotional difficulties and who can present complex and challenging behaviours.

The role involves providing support in emergency and crisis situations to young people across Worcestershire, which includes working evenings and weekends. We are looking to appoint staff who are committed and skilled in working with young people and who can empower young people to develop positive change within their lives.
