Are you a social worker with Continuing Health Care Experience? We are looking for enthusiastic, dynamic and adaptable individuals to work within our Continuing Health Care Social Work Team for Worcestershire County Council. We have a number of permanent 37 hours per week positions based in the Worcestershire Area.
To be successful in this post you will need significant experience as a level 3 Social Worker with an understanding of the National Framework for NHS Continuing Health Care and NHS- Funded Nursing Care (October 2018 (revised) Guidance. The suitable candidate will be well supported within their role.
Or if you are a social worker with experience in Area Social Work, we have a vacancy based in Kidderminster: supporting people who are over 18 with a wide range of support needs (excluding Learning Disabilities and Adult Mental Health). We work closely with GPs and other community health colleagues to develop models of integrated working, called Neighbourhood Teams.
In Worcestershire, we use the Three Conversation approach, a strength-based model that focuses on people's strengths, own assets and those in communities. All our staff receive support from Advanced Social Work Professionals, have regular supervision, opportunities for team reflection and development on a weekly basis. Many of these roles can be worked flexibly and with mobile working. Plus, on top of our generous annual leave the opportunity to buy extra holiday. You will be part of a service keen to develop your career with a supportive manager who provides regular supervision.
