Salary G9 £32,552- £37,475 per annum

This is an exciting time to join Walsall Council’s Adult Learning Disabilities and Transition Team.

We are a dynamic, welcoming team working with adults with Learning Disabilities, their carers and families. We support people from age 18 years onwards and work with children’s services supporting young people with Learning Disabilities (in Transition we also work with Physical Disabilities and Autism) from age 16/17 years transitioning to adulthood.

The team is made up of Team Manager, 3 Advanced Practitioners, Social Workers, Social Care Facilitators and an Occupational Therapist. We undertake assessments, support planning and reviews under the Care Act 2014, working closely with Health partners, Education; SEN, Schools and Colleges, Children’s Services, Transition and Leaving Care Workers, Commissioners and other external agencies in order to provide an integrated approach to supporting individuals.  Working in a strengths based and risk enabling manner to achieve best outcomes and increase their independence.

We currently have a permanent full time vacancy for an experienced Social Worker to join our team. We are looking for an enthusiastic and committed professional to join our team, to help shape and deliver our service, and contribute to the development of a model of best practice.

The successful applicant will be skilled in carrying out strengths based assessments and support planning under the Care Act 2014, completing comprehensive risk assessments, Mental Capacity assessments and Best Interest Decision Making. You should have experience of working within a multi-disciplinary setting, to include health partners and voluntary and community organisations.

We are looking for a forward thinking team member who will promote and deliver positive outcomes for individuals and their carers. Therefore, you will need to demonstrate significant experience of working with people who have Learning Disabilities.  Integral to this role is having a comprehensive working knowledge and understanding of Making Safeguarding Personal and experience of undertaking safeguarding enquiries.

In return, we offer a supportive environment, where every level of the organisation is committed to ongoing practice development.  We are committed to improving outcomes for service users and your continued professional development.
