Job Details
An exciting opportunity has arisen for a Case Worker within the Children with Disabilities Team.
This is an opportunity to join a service that is reshaping our approach to delivery of support through the child’s journey. This will be achieved through working with children and their families to translate identified social care needs into a practical network of support, through a Personal Budget which will achieve positive outcomes.
The post holder will be responsible for the planning and review of cases where the child/young person has additional education and/or social care needs. The children/young people’s needs will be met through a multi-disciplinary team. These children may have an Education, Health and Care plan or they may be undergoing assessment relating to additional or special educational needs. The children/young people may also have disabilities and/or complex health needs.
The post holder will work directly with children/young people and their families to ensure their views are listened to, and that they are fully engaged in processes to ensure appropriate support is implemented and reviewed. They will also be a point of contact for families and will provide information and advice as appropriate.

You will become part of a supportive team and will receive regular, good quality, supervision and excellent training and development opportunities. The post requires experience and knowledge of working with children with special educational needs and/or disabled children and their families. Literacy and numerous qualifications e.g. GSCE – English, Maths A-C or NVQ 3 in childcare or equivalent.
The team are committed to high standards and to achieving the very best outcomes for children and young people through proactive and child focused support planning, which will be supported by reflective social work groups, and by our close multi-agency working arrangements with all relevant agencies within Telford & Wrekin. Above all we value and listen to the views of children and young people and shape our services around their needs.
