Assistant Care Co-ordinator
An excellent opportunity has arisen to work in the Assessment and Independence Team as an Assistant Care Co-ordinator
Hours: 37 per week
Salary: Grade 6
Location: Fixed term contract based at Russell's Hall Hospital and/or Cottage Street, Brierley Hill until 31 March 2020 in the first instance.
Your primary role will involve working alongside social work colleagues to provide support in assessing vulnerable adults (either in the community or within a hospital setting) to contribute to the development of individual support plans. These plans enable people to achieve the optimum level of independence, to maintain relationships, and be valued members of their community. You may also be required to undertake responsive assessments for people in crisis in the community.
You will need to be able to work under pressure and prioritise your own workload. You will also need to have professional commitment and a high level of knowledge, critical thinking, communication skills and creativity.
You will be working 37 hours per week on a set rota which includes weekends, evenings and bank holidays, for which relevant enhancements will be paid.
You may need to work flexibly, travelling across the Dudley borough. You will therefore need to hold a valid full driving licence and have the use of a vehicle.
The post is subject to medical clearance and an enhanced DBS check.
