Dudley Council has been transforming the way it delivers adult social care and is investing in a multi-million pound package of adult social care and health initiatives for the most vulnerable.
In 2017, the council was awarded £13.9million to spend until March 2020 as part of the government’s £2billion Improved Better Care Fund. The majority of the funding has been used to introduce initiatives to help meet the increasing demands on adult social care and reduce pressures on the National Health Service.
We now have 2 fixed term posts to 31 March 2020 to help with this transformation of support for our ageing and disabled population.
Hours: 37 per week
Location: Fixed Term Temporary Posts based at Russells Hall Hospital and/or Cottage Street
You will be working in the Assessment and Independence Team to help prevent hospital admissions. Your primary role will involve working alongside social work colleagues to provide support in assessing vulnerable adults within a hospital or home setting to contribute to the development of individual support plans. These plans enable people to achieve the optimum level of independence, to maintain relationships, and be valued members of their community. You may also be required to undertake responsive assessments for people in crisis in the community.
You will need to be able to work under pressure and prioritise your own workload. You will also need to have professional commitment and a high level of knowledge, critical thinking, communication skills and creativity.
You will be working 37 hours per week on a set rota which includes weekends, evenings and some bank holidays, for which relevant enhancements will be paid.
You will need to work flexibly, travelling across the Dudley borough. You will therefore need to hold a valid full driving licence and have the use of a vehicle.
The post is subject to medical clearance and an enhanced DBS check.
