The Edge of Care Service are recruiting to the position of full-time permanent Team co-ordinator based in Stafford, but working across the county.
The Edge of Care service incorporates the Intensive Prevention Service which went live in January 2013. The main responsibilities within this post is to offer management support within the Intensive Prevention Service, however there is an expectation that management support will be provided across the service as a whole.
The service has been successful in keeping young people at home and improving their outcomes and has expanded over time to support young people returning from local authority care.
The service is based around a model of intensive one-to-one key work over a sustained period targeted to meet the specific needs of the client.
The key worker will then be responsible for working with young people and their parents to develop a tailored package of support, drawing in and coordinating other resources within the team as appropriate. The key worker will also support young people and parents to identify and engage with any relevant external support services as required e.g. substance use services, housing providers etc.
You will have responsibility for managing referrals, allocations, supervision of Senior Family Support Workers, quality and performance within the service.
You will need a Social Work Qualification and have at least two years post qualification experience in Children’s Services.
