Sandwell Young Carers is a registered charity providing services to children and young people, living in Sandwell, whose lives are affected as a result of caring for a dependent family member.

We are looking to recruit a Young Carers Transition Worker. 

Transition Worker - £9,620 (20 hours per week)

The post holder will be responsible to our Support Work Co-ordinator and will be required to provide a support service to young carers who are transitioning into adulthood. Those who are young adult carers (aged 14-24 years), will need support with a focus on their Education, Training and Employment needs and aspirations.

You will need to assess the individual needs of those referred and assess or provide appropriate support that will address both physical and emotional well-being and promote meaningful and positive outcomes.
