Job Details
Do you wish to develop additional skills and understanding of the wider health and social care landscape? This role can provide an exciting opportunity to enhance and develop your existing knowledge base.
The Continuing Healthcare Team within Coventry City Council works closely with colleague in the NHS, participating in assessment and reviews for people with complex health and social care needs. The work of the team is varied and team members have a close working relationship with Health colleagues, particularly from the local Clinical Commissioning Group. The CHC team also supports social care practitioners and managers throughout the Local Authority, processing all referrals in from the CCG in relation to NHS Continuing Healthcare.
You will work with team practitioners and the Lead Officer for Continuing Healthcare, to support the processing of referrals sent to the team daily by NHS colleagues. You will liaise with colleagues both within the Local Authority, but also the NHS, to ensure that information received is accurate and actioned in a timely manner. The role will also involve supporting the Lead Officer in relation to the Continuing Healthcare Ratification Panel and other joint work with the Clinical Commissioning Group.
You will understand the importance of the relationship and joint working between the Local Authority and the NHS. Have a positive attitude towards building constructive relationships within the LA and with other agencies, and value the importance of a supportive team environment.
Familiarity with Care Director would be an advantage along with an understanding of the role of Adult social care and the difference between the Local Authority and Health remit. Ability to work within a team and contribute towards supportive working in situations that can be fast-paced and sometimes stressful.
You will also have a knowledge/experience of adult social care and understand the way in which the Local Authority and the NHS work together, locally, but it is not expected that applicants will have a detailed understanding of NHS Continuing Healthcare as training will be provided for the successful candidate.
