Warwickshire County Council has an exciting opportunity to recruit a full time Occupational Therapy Assistant to join the Reablement team.  The post will be based in the South (Heathcote House, Leamington) The full-time post will be for 37 hours per week.  The post requires you to work a shift pattern of 9am- 5pm and 12pm – 8 pm on weekdays, one Saturday in every 7 weeks with the possibility of this increasing to Sunday's depending on demand of the service. This is done on a rota basis and a day off will be given in the week to take into account working Saturday and/or Sunday. There will also be a requirement to work bank holidays on a rota basis. This can be discussed in more detail at interview. There will be an expectation that the post holder will be required to provide countywide support to both the North and South Reablement teams.

 

 

Reablement focuses on promoting independence by supporting people to do as much as possible for themselves both physically and psychologically. It aims to do this by supporting people to continue living at home, preventing admissions into hospital or residential settings, and facilitating timely discharges.

 

 

We believe that by supporting customers to identify and achieve their personal goals and outcomes, and by embedding personalisation, customers will have choice and control over how their support will be delivered, this will have a positive impact on health and wellbeing.

 

 

Reablement is a short-term service working with customers for up to 6 weeks.

 

 

The Role

 

As an Occupational Therapy Assistant, you will be working with customers in their own homes to help them achieve independence with daily living tasks. You will be working closely with Occupational Therapists, Reablement Officers & Reablement Assistants to engage customers with functional assessments promoting their independence. You will be the first to visit a customer and together you will complete an initial assessment, risk assessments and identify the customer’s daily living tasks that can be reabled.  You will create a support plan where Reablement Assistants will support the customer to achieve their outcomes which are reviewed during the service. You will also assess for and prescribe items of equipment to enable independence and reduce the need for ongoing support. You will be responsible for managing your time and work load and will be expected to work closely with your colleagues, as well as independently.

 

 

Key Requirements

 

Excellent communication skills and time management skills are needed due to the fast pace nature of the service.

 

Knowledge and experience of using a range of equipment to support functional ability is desired.

 

Computer skills are important as we use a range of databases/computer programmes to communicate and document our interventions

 

The role requires you to liaise with colleagues, customers, health professionals and the public, either face to face or over the telephone, so it is important you have a courteous, professional manner.

 

You must be able to work as part of a team but also have the ability to work on your own initiative.

 

You must be able to drive and support the service both South and North of the County.

 

It is important to respect and maintain confidentiality of information, whilst adhering to Warwickshire County Council Policy and Procedures.

 

You should maintain a commitment to anti-discriminatory practice in employment and service provision.

 

Satisfactory completion of an enhanced check through the Disclosure and Barring Service is required (this will be taken up if offered the post).
