12-month fixed term/secondment full-time (37 hours per week) SEND Key Worker post to start in January 2021 based at Staffordshire Place 2, Tipping Street, Stafford ST16 2DH

Staffordshire County Council is seeking to appoint one full time temporary SEND Key Worker within the Stafford and South Staffordshire District Office, SEND Assessment and Planning Service to co-ordinate new education health and care (EHC) needs assessments, maintain and review EHC plans and undertake any other duties that relate to the EHC process.  This particular post focuses on Preparation for Adulthood through co-ordinating the Council’s involvement and response to EHC reviews of children and young people that are Y9 or older and will cover the South Staffordshire area.  The post is of a temporary nature in order to cover a maternity leave.

We are seeking a passionate, motivated individual who has a real drive to work with children and young people who have special educational needs, their families and schools, in a truly person-centred way that fully involves the child, young person and their family in the process.  Developing collaborative relationships with individuals with SEND of all ages and their families, that promote choice and independence, respecting diversity and difference and reach agreement on appropriate, tangible and realistic outcomes is an integral part of the SEND Key Worker role.  Furthermore, you will be fully expected to collaborate with other professionals, both within Staffordshire County Council and external partners, such as schools, Virtual School, the Children Disability Service, Commissioner, Inclusion Officers, Safeguarding, NHS professionals, SENDIASS etc.  This is an exciting opportunity to join a service that truly aims to make a difference to the outcomes of children and young people with SEND in supporting them to fulfil their aspirations and preparing them for living independently, gaining meaningful employment, being part of the community and maintaining good health.
