This role is critical in ensuring we embed The Three Conversation model of strengths-based practice and promote cultural change to ensure that this approach continues to deliver a better service for the people we work with, our teams and our partners.

The job will require you to support social work teams across all service areas and in supporting other areas of work that have a direct link with Adult Social Care.

Working with staff and managers you will:

  • Be part of Quality & Safeguarding Services, working closely with the Practice Development Lead to deliver key objectives;
  • Support Adult Social Care staff to continuously develop The Three Conversations approach;
  • Identify and promote best practice in social work teams;
  • Review and amend existing policies and practice guidance to reflect a strengths-based approach
  • Work with colleagues on service developments to ensure there is a seamless service for residents that compliments the strengths based model, such as the development of an Integrated Wellbeing  Offer (Front door), Integrated Care System (Health and social care working much more closely together) and the use of Liquid Logic (Adult Social Care recording system) to support strength based working.
