About Us
Warwickshire Children with Disabilities Team are seeking 2 Family Support Workers to join the team on a permanent, full-time basis.
Focusing on strengths and utilising an asset-based approach, you will support and enable children and their family members to reach identified outcomes to meet their full potential and maximise the child / young person’s independence.
The team is county wide and based across two sites – one in the north of the county and one in the south. The role will require you to be prepared to work county wide as required.
The Role
37 Hours per week
The Family Support Worker roles will be based within the Children with Disabilities Review Hub. These roles will involve:
- Managing a caseload of allocated children who receive a support package which meets identified needs.
- Review packages of support, and work with internal and external colleagues and parents / carers to ensure support packages remain effective and that positive outcomes are achieved.
- Co-ordinate and chair multi-agency review meetings and ensure children and their families are seen in order to review need.
- Provide appropriate signposting and support to parent / carers
- Working alongside social workers on duty.
- Undertake Carers Assessments.
