To provide assessments and support plans to parents of children with disabilities and/or additional needs and children and young people aged 5 – 25 who are caring.  You will explore the impact of caring on their lives through assessments, keeping their physical, mental and emotional wellbeing at the heart of your practice.  Further to this, you will produce support plans that meet their needs and enable equal access to fulfilled lives.

The project exists within the young carers team and aims to increase networks of support, confidence and resilience for carers through self-directed and nurturing support.  It also aims for parent carers, young, young adults to be parents, children and young people first.

Functions specific to this role

 

  1. Deliver Assessments, Support Plans and Reviews including Transition and Care Act compliant Carers Assessments to parents, children and young adults whose lives are affected by caring for someone in Solihull. Engage and work positively in partnership with health and social care, community settings, third sector, families and the market of providers in Solihull to maximise choice, control, flexibility and independence for carers. Attend relevant multi agency meetings to ensure effective partnership working Offer personalised support to carers to access and/or remain in employment, education or training To support transitions from children to adult services, education, training, employment and post caring Consider eligibility for care and support services for the cared for Support carers to overcome barriers and exercise their rights to achieve their desired goals Link your work with other services from Carers Trust Solihull and community based support Operate under Children and Families Act 2014 and Care Act 2014 legal frameworks Maintain accurate records of contact and evidence outcomes of your work Apply innovative practice to maximise experience of assessments and support plans for carers, including social media and assistive technology Contribute to the marketing of services for carers in Solihull and in the development of resources and tools that enable carers to access them e.g. website, information packs Identify and respond appropriately to safeguarding issues Attend regular external and internal meetings to determine ways forward for the team in delivering assessments for parents, children and young people. Contribute to the publicity and awareness raising in your area of work and the work of the organisation Share in the cover of reception duties for the whole organisation and duty for young carers team as and when required Any other duties as agreed by the Chief Officer
