An exciting opportunity has arisen to join Intermediate Care Service in Walsall, this is a multi-disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care.

We are particularly looking for an enthusiastic and committed individuals to join our team to help shape and deliver our services, and contribute to the model of best practice. The successful candidates will be required to have experience of working in Adult Social Care and in a Multi-Disciplinary Team. 

The Integrated team is made up of a Service Manager, Team Manager, Advanced Practitioners, Social Workers, Social Care Facilitators, Occupational Therapist, Physiotherapy, Nurses and Rapid Response. We undertake assessments, support planning and reviews under the Care Act 2014, working closely with Health partners, Commissioners and many external agencies in order to provide an integrated approach to supporting individuals to achieve good outcomes. 

The successful applicants will be skilled in carrying out strengths based assessments and support planning under the Care Act 2014, completing comprehensive risk assessments, Mental Capacity assessments and Best Interest Decision Making. You are required to have experience of working within a multi-disciplinary setting. Integral to this role is having a comprehensive working knowledge and understanding of Making Safeguarding Personal and experience of undertaking safeguarding enquiries.
