25 hours per week – 5 hours per day
Fixed Term until 31 March 2022
We are working in partnership with Warwickshire County Council to identify and resolve housing issues that could delay a patient’s discharge from hospital.
The post holder will be working with health and social care teams across hospital settings and the wider community to identify and implement solutions to individual challenges.
The person we are seeking will have;
• Experience of working with clients who have a variety of complex support needs
• The ability to work in partnership with others to identify barriers and implement solutions
• An understanding of the housing issues that can prevent a patient being discharged from hospital
There will be a requirement to work at a variety of sites across the County, although the main base will be in Rugby.
