Salary G7 £26,773 - £31,037 per annum
3 permanent full-time posts
An exciting opportunity has arisen to join Intermediate Care Service in Walsall, this is a multi-disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care.
We are particularly looking for enthusiastic and committed individuals to join our team to help shape and deliver our services, and contribute to the model of best practice. The successful candidates will be required to have experience of working in Adult Social Care and to join our Multi-Disciplinary Team.
You will need to demonstrate a good knowledge base of the Care Act 2014 principles, and be able to work autonomously in the community to support service users and their carer’s in completing home visits, assessments/reviews, support planning and carer’s assessments. You need to have an awareness of safeguarding principles and responsibilities under the Care Act. You need to have a good practice knowledge base of the Mental Capacity Act and the Deprivation of Liberty Safeguarding framework.
In return, we shall offer you access to a supportive Team Manager and Advanced Practitioners, regular supervision and good peer support. You will have an opportunity to enhance and develop your skills, as Walsall offers a range of training and development opportunities.
