Job Details
Sandwell Adult Social Care Health and Well-being are seeking a Social Worker for our borough wide hospital/intermediate care service.
The service aims to improve the outcomes of residents in Sandwell by;
- Enabling/facilitating timely discharges from hospital
- Monitoring and facilitating timely discharges from intermediate care provisions
- Offering alternatives to hospital admissions
- Supporting people to return to independent living
You will deliver an enhanced customer experience that promotes the wellbeing of vulnerable adults to enable them to live independently for longer, supporting them in retaining and regaining their skills and confidence.
You will be expected to manage a varied case load which includes access to Service Users who are COVID positive within the community within their own home, residential/Nursing care home settings and Hospital wards – to complete Face to Face assessments.
You will be social work qualified and hold either the degree in social work, Dip SW, CQSW or equivalent and SW England registered.
An Enhanced DBS will be required for this role
You will be expected to be office and home based dependent upon a rota, if you are office based you will be required to work between 2 hubs which are Sandwell and Rowley Regis Hospital, you will also be required to work out in the community as and when required.
The team comply with the government’s guidance of managing the risk of Covid-19, you will receive training on Covid-19 safeguarding procedures.
The following PPE, which includes Face shields, Gloves, Aprons and Hand sanitiser will be provided for the role, we are also in the process of setting up FFP3 Masks.
