As an exemplary practitioner, you will be required to provide support, advice and share professional knowledge to colleagues; promote innovation and model and facilitate reflective practice.



Hours:  1 post x 37 hours, 

Salary:  Grade 8

Location:  Cottage Street, Brierley Hill or Russells Hall Hospital/home

The post will involve working with adults and this may involve vulnerable adults. You will be requried to undertake assessments and develop support plans to enable them to achieve an optimum level of independence to develop and maintain relationships and be valued members of their community.

Due to the requirement of the setting, you will be asked to work flexible hours based on a 7-day rota covering weekends and bank holidays.  

Demonstrate professional commitment and a high level of knowledge, critical thinking, communication skills and creativity, in order to support people to achieve positive outcomes and a good quality of life.

As a social worker, you will be involved in working with multi-agencies to facilitate safe discharge following different pathways as necessary.

You will also be asked to undertake duty and respond to front of house to facilitate immediate discharges for those who are not liable to be admitted.   

We are committed to safeguarding and promoting the welfare of vulnerable adults; therefore the post is subject to medical clearance and a enhanced DBS check.
