About Us
We are seeking energetic and committed, experienced social workers to help young people with disabilities make the transition to greater independence and opportunities as they grow older.
Play a part in making Warwickshire the best it can be, working with people aged 14 to 25 in the Disability Transitions Team. It’s a big responsibility that can be challenging at times, but that is what drives us to succeed, and you’ll see their independence and your career grow.
The Disability Transitions Team works with dedicated passion and purpose to provide specialist care and support to the most vulnerable in our communities. Working in our team, you’ll see just how much of a meaningful difference you can make. You will be working with a variety of young people, including those with learning disabilities and autism, and/or physical disabilities, their carers and their families.
The Role
You’ll work closely with schools, communities and our education and health partners. We’ll attach you to one of our schools or colleges so you can get to know all the pupils, the local area, community resources and services, so you can develop friendship networks and use pooled resources to help young people achieve their full potential.
