About Us

An exciting opportunity has become available within Warwickshire County Council’s Personalisation Team for an Occupational Therapist Assistant. The position is for a 12-month fixed-term contract.

The position will be covering north and south of the county and the full-time position will be for 37 hours per week and Job share, part-time can be considered.

Working as an Occupational Therapist Assistant your role will be to conduct assessments with customers who are struggling to manage activities of daily living in their homes and who have long term health needs that require support.

You will assess the customer needs taking a strengths-based approach and enable the customer to engage and identify their skills or provide compensatory equipment needed so that they can function more independently.

You will empower the customer and will identify ongoing areas of need and refer onto the appropriate service(s) as required. You will work as part of a team that consists of:

Occupational Therapists, Occupational Therapy Assistants, Social Care Practitioners and Social Workers.

You will also provide professional support and guidance to the team and you will receive professional support from Occupational Therapists and Team Leads. You will work within the requirements of the care Act 2014, HCPC codes of practice and other legislation and guidance.

Personalisation focuses on promoting independence by supporting customers to do as much as possible for themselves both physically and psychologically. Personalisation aims to do this by supporting customers to continue living in their own home, preventing admissions into hospital or residential settings, with the use of equipment, compensatory techniques and adaptation to their home, whilst utilising the customers strengths.

We believe that by supporting customers to identify and achieve their personal goals and outcomes, and by embedding personalisation, customers will have choice

and control over how their support will be delivered, this will have a positive impact on health and well-being.

The Role

You will be working within the service to respond preventatively to referrals for social care and support and will use Occupational Therapy interventions and a strengths-based approach to enable adults and older adults to remain in their own homes with the optimum level of independence and to enable / support customers to achieve their desired outcomes.

You will be responsible for managing your time and workload and will be expected to work closely with your colleagues within the team and external agencies /services, as well as independently.

You will also act as a point of expertise to the wider multidisciplinary team, providing support on complex cases and carrying out joint assessments.
