Population Health Management Analyst - Health & Social Care
PO4: £33,782 - £36,922 per annum
37 Hours per week (permanent position)
Are you passionate about evidence based decision making? Do you understand how data can drive improvements in health outcomes?
To support the delivery of a shared vision to improve the health of the Borough’s population, Telford & Wrekin Council and Shropshire, Telford & Wrekin CCG are recruiting to a new joint role that will lead the development of a joint Population Health Management strategy and implementation plan. This will drive the development of shared performance, business and community intelligence products to inform commissioning decisions.
You will need to demonstrate:
- An understanding of the principles of population health management and its scope for driving change
- An ability to lead and drive forwards work programmes to embed evidence based decision making in complex organisations
- Experience of working in an environment of continual system change and understands the implication this has for the generation of business intelligence.
- Ability to analyse, interpret and report on complex datasets using appropriate analysis and statistical techniques

The post will be based in the Insight Team of Telford & Wrekin Council but will work very closely with the CCG. The successful applicant can be employed by either Telford & Wrekin Council or Shropshire, Telford & Wrekin CCG.
