Personalised Support Officer (Grade 5)


 


We are seeking a Personalised Support Officer to join the Personalised Support Team within Community Financial Support Team in a temporary position to 31st March 2022.


The successful applicant will join a team of Personalised Support Officers to ensure that services are person centred, outcome focused, inclusive and offer value for money.


The post holder will need to demonstrate an ability to work in a fast-paced environment with constant change, show a willing and positive attitude and ideally, you will have good understanding and experience of working within Adult Social Care


This role will have a particular focus on supporting with HomeFirst and reablement packages and also 24-hour Care to enable individuals to remain in their home environment and live independently as possible.


The post holders will:



  • Have a good understanding of social care processes, policies and provision


 



  • The ability to communicate effectively in often challenging circumstances is essential


 



  • Liaise and negotiate with service providers i.e. negotiate care packages times and placement costs


 



  • Negotiate care home placements


 



  • Ensure the Provider works with the person to achieve the desired outcomes


 



  • Be able to take a problem-solving approach to tasks and work to strict timescales


 




  • Have a good level of IT skills with CareFirst system experience desirable


 



 


A test will be sent to the short-listed candidates on 19th May for return to dawn.williams2@wolverhampton.gov.uk by 12 noon on 20th May. Interviews will be held on 21st May.
