The day-to-day duties

The Bridges Senior Care Co-Ordinator will have a supervisory element to their role and will be responsible for line managing the Bridges Care Co-Ordinator and Administrators of the Bridges team.

The Senior Care Co-Ordinator will support in the operational delivery of the Bridges Service across Birmingham, supporting people living with cancer and other life limiting diseases across Birmingham and surrounding areas, to maximise their quality of life at home.

The Senior Care Co-Ordinator will work alongside other team members and will be responsible for taking referrals, visiting clients, assessing needs, effectively organising and coordinating services/support, navigating client to other appropriate support/services, advocating on their behalf or accompanying them on their cancer/palliative care journey.

Knowledge, Skills and Qualifications required:

The successful candidate will have experience of community development and outreach working within the community and experience of working with members of the public in a health and social environment. They will also have experience of managing staff and working directly with services users or other vulnerable groups. The Senior Care Co-Ordinator will have experience of working in a multi-disciplinary environment and experience of working with other partner organisations and will work as an effective team member.

The successful candidate must demonstrate organisational and time management skills, to prioritise workload and work on own initiative and must be able to manage boundaries with both service user and professionals.

If required must be prepared to work flexibly, as the needs of the service demands. Must have access to own transport and be willing to travel to Sandwell and across Birmingham.
