32234 to £34728 dependent on experience

 

We have two locality teams based in our communities working with older people, physically disabled and those with an acquired brain injury.

We use strengths and asset based approach in our practice; how individuals can be supported by the strengths they have in themselves, their network of support from family, friends and their local communities. Where care and support is required we work to ensure that this reflects the individual’s assessed needs and strengths while promoting their independence, choice and control.

We encourage positive risk taking with a ‘home is best’ philosophy, this is reflected in our performance as Telford and Wrekin is proud to have a lower than national average per population of admissions into long term placements for older people.

The Social Worker roles are expected to undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act.  To undertake duties expected of Social Workers in line with the Professional Capabilities Framework and in accordance with the specific job description attached. The Social Work roles will work with complex Adults, their carers/family as directed by the Team Leader
