Reporting to the Lead Commissioner of Commissioning and Partnerships
(with professional supervision from Quality & Safeguarding Services)

The health and wellbeing of our residents is a big priority for us here at the County Council.
Using Here2Help as a dedicated wellbeing hub, we want to encourage a community response for people in need of health and wellbeing support with the aim of focusing on the strengths of the individuals and the support network around them.
 
This is an exciting opportunity for someone who is passionate about improving the lives of the residents of Worcestershire and wants to make a significant contribution to a change programme which is person centred and needs led focussed. This role is for someone with a genuine passion for strength-based working to join and work with the Communities and Partnership team to build on the continued success of the Here2Help service.
The Advanced Practitioner will bring their practice expertise working with frontline staff, managers and partners to design, develop and implement the new integrated wellbeing service offer; Here2Help. 

You will be an important part of the Programme Delivery Team and will be expected to lead transformational change in ways that deliver the expected value by acting as the conduit between the business areas and the programme.
You will join the team to operate across the programme workstreams to support the vision for Here2Help by leading change management initiatives within your respective areas of expertise.

Key skills and experience:
Significant operational experience in either adults or children’s services and the ability to communicate well with your peers. You need to have significant experience of service improvement development/change initiatives with successful outcomes.

The post requires the ability to identify and implement new ways of working to achieve positive outcomes.  You also need to demonstrate the ability to interpret complex information and to present findings, insight and recommendations to the workforce in your area and the programme team.
It is essential that the postholder has a professional qualification in social work e.g. Social Work Degree, DipSW, CQSW or Youth work or working in the community qualification, NVQ, City & Guilds or other equivalent qualification.
